  • Eden - Pambula

MAIN DUTIES/RESPONSIBILITIES: 

  • Providing home care support to clients in their home
  • To develop appropriate, supportive working relationships with clients and their families/carers.
  • May include Personal Care, Social and Community Access, Domestic Assistance and transport 
  • To provide support which reflects the Disability Services Standards, legislative requirements of our In Home Care Services
  • To ensure all client case notes and client information is updated during or soon after each interaction or shift with our clients 
  • Maintain a clean and safe workspace, including your own vehicle 
  • To actively promote SS2YOU Vision, Mission and Values in everyday work and in the community
  • Abide by workplace health and safety policies and procedures and follow, contribute and work in a culture committed to continuous improvement
  • Stay up to date with relevant policy and procedures relating to your role
  • Commit to ongoing development and training
  • To participate in supervision and team meetings as directed by your manager
  • To meet your ongoing training, quality and development needs
  • Other tasks as directed 

OBJECTIVES OF THE ROLE

  • Workers are directly responsible for the day-to-day support, and personal development of clients accessing the services provided by SS2YOU.
  • Workers ensure that they support clients to be at the centre of their decision making, to enable our clients to achieve the desired outcomes of their care plans, goals and aspirations in partnership with their families and carers
  • Workers ensure that individual and group supports provided to clients are provided in accordance with the Aged Care and National Disability Services Standards and other relevant legislation
  • Worker’s will ensure they are active in ensuring the rights of people are upheld and will always act in the best interest of their clients
  • To perform or assist in a Key Worker role and undertake buddy shifts if applicable to you and as required
  • To work together as a team to assist our clients to achieve their goals

SKILLS & EXPERIENCE 

Qualifications Skills and Experience:  
Essential:

  • Experience working with vulnerable people in a role which would allow transferable skills or appropriate lived experience
  • A passion for supporting people with a disability/health condition that impacts their capacity to have the best life they possibly can, by considering clients full human rights and making them feel valued as part of their local community
  • Demonstrated capacity to work with honesty and integrity
  • Good literacy, numeracy skills
  • Practical written and verbal communication skills, including the capacity to apply policy and procedure
  • Organised and able to meet deadlines
  • Solid computer skills with the ability to use online platforms
  • Possession of a current driver’s license
  • Possession of a current First Aid Certificate (within 3 years)
  • Possession of a current CPR Certificate (within 1 year)
  • Satisfactory criminal history check - within 3 years
  • Possession of a cleared Working With Children Check (Paid worker) within 5 years


Qualifications and Experience: 
Desirable:

  • Certificate III or IV in relevant studies and or a willingness and capacity to undertake further studies

  • PERFORMANCE GOALS: 

    • Undertake client supports in a person centred way.
    • Deal with clients, families, carers and other stakeholders professionally at all times 
    • Ensure your vehicle is clean and presentable at all times for the purpose of transporting clients
    • Meet the Vision, Mission and Values of our organisation.
